Why this role matters
Specialist Homelessness Services (SHS) operates within the National Homelessness Stream of The Salvation Army (TSA) and is situated across multiple sites with the Wimmera South West Region.
About the role
You will build on your homelessness knowledge and experience to advance the issue of homelessness within the Wimmera South West. As a motivated and proactive individual you will provide assessment, information and referral, support, case management, counseling for clients who are socially isolated, homeless or at risk of homelessness.
You will successfully
- Undertake intensive client assessment for intake and the provision of information and referral
- Attend to clients on an outreach basis and attendance at alternative sites
- Coordinate supports through case management practices, and referral to key support agencies
- Orientation, and induction of new clients
- Provide and offer clients suitable housing that will enhance their ability to work towards their goals
- Develop appropriate and holistic case plans for all clients with a strength based, solution focussed approach
- Develop appropriate and realistic housing plan for all clients participating in the program to assist clients in making informed housing decisions
- Assist clients to access appropriate day activities, training and development programs
- Link clients into job networks and other vocation programs
- Answer telephone enquiries about TSA Homelessness Hamilton service delivery
This is a part time permanent position 21.7 hours per week based in the Horsham office. The Outreach position services Horsham and Hindmarsh LGA’s. The Hindmarsh outposts are to be visited twice a month and as needed basis.
Salary and conditions are in accordance with Social, Community, Home Care and Disability Services Award/General Retail Industry Award, Level 5

You will have
- Demonstrates knowledge of homelessness including context, causes, issues, philosophies and legislation
- Understands and applies homelessness models, approaches, theories and practices when dealing with clients
- Ability to understand and respond to the needs of individuals experiencing homelessness or at risk of homelessness
- Social Work qualification or other relevant social tertiary qualifications and/or substantial experience
- A current VIC Drivers Licence
