About the role – Permanent Full Time/max term contract until June 2025
The Community Connections program is funded by the Department of Human Services (DHS) which aims to support people to create and work toward goals around social engagement and build relationships in their community in the Eastern, Northern and Southern region of Adelaide Target participants are vulnerable people aged 18 – 64 (or 49 years for Aboriginal people) living in social isolation, with the aim to engage with, and participate in community activities, groups and volunteering opportunities.
The Intake, Assessment & Data Officer will provide the initial intake and assessment of program participants, complying with all DHS and Baptist Care SA reporting and documentation requirements, before assigning clients to a case manager in consultation with the manager and other staff. All work is to be completed in line with Baptist Care SA’s Tapa Marnirni-Apinthi Practice Framework.
Key responsibilities will include, but will not be limited to:
- Complete initial intake and assessment of program participants across the three regions, in line with the Baptist Care SA practice framework requirements
- Develop appropriate case plan for clients, reflecting client needs and expected outcomes.
- Identify and open new referral pathways by building relationships with relevant services in the service region
- Assist with the monitoring and evaluation of service through qualitative and quantitative data, case studies and sector support to ensure maximum outcomes for clients, effective service delivery, program targets met and programs are sustainable and attract ongoing funding.
About you:

Your passion for helping others is evident in your commitment to providing excellent client services. You possess strong analytical skills and are adept at using data to drive decision-making. As a collaborative team player, you excel in communication and enjoy working in an environment that values diversity and inclusion.
You will also have the following skills and experience:
- Tertiary qualifications in Human Services (e.g. Social Work, Youth Work, Psychology, Community Development) or Diploma in a related field of study.
- Extensive relevant experience (at least 2 years) may be considered in lieu of qualifications
- Experience in working with Aboriginal people and communities in the provision of support services to people who are disadvantaged, to families, and has a good understanding of Aboriginal, multicultural and social justice issues as they impact on service delivery
- Solid understanding of community development principles and social justice principles

You will be required to have, or be willing to obtain:
- Full unrestricted South Australia Driver’s Licence
- A current and valid DHS: Child-Related Employment Screening/WWCC
- National Police Clearance
- Training: Provide First Aid (HLTAID011)
- Training: Safe Environments for Children and Young People (Through Their Eyes)
- Ability and willingness to travel within South Australia as required
- A fully insured and roadworthy compliant vehicle (roadworthy check required)
