About the opportunity

The role of the Intake Service Navigator exists to improve accessibility to the health system for individuals with severe mental illness and associated psychosocial disorders. The Intake Service Navigator will also be one of the main points of entry, intake and assessment for psychosocial supports across the Gippsland region.

Working closely Local Health Networks and NDIS Local Area Coordinators the Service Navigator will work to facilitate consumer access and enhance coordinated referral processes through the creation and distribution of detailed resources relating to community-based support services as well as psychosocial, clinical and primary healthcare.

The successful applicant will work alongside Psychosocial Support Services and reports directly to the Senior Intake Service Navigator.As Intake Service Navigator you will engage with and advocate for individuals, connect them with the appropriate support networks and empower them on their recovery journey.Building and drawing upon Wellways’ Community Recovery Model, the Intake Service Navigator will establish and enhance coordinated referral processes as well as community support services focused on psychosocial support needs such as housing, employment, education, family support and financial matters.

To be successful in this role

  • Relevant qualification or commensurate experience in working in a support role for people with mental illness, generally a Diploma IV qualification or above
  • Lived experience of Mental Health Recovery
  • Current valid Driver’s License and the ability to undertake travel for the role
  • Current Working with Children Check and NDIS Workers Screening Check
  • Right to Work within Australia
  • Satisfactory National Police Records Check
  • NDIS Worker Orientation Module Certificate
  • Evidence of COVID vaccination with booster or valid medical exemption
