JOB DESCRIPTION

Uniting Recovery delivers a range of specialist clinical programs supported by multidisciplinary teams. These programs are located across Sydney and include youth, adult and older persons mental health.

The Integrated Primary Care Pilot Project is a new model of primary mental health and addictions care and support, which aims to provide all people experiencing mental distress or addictions challenges with access to convenient, high quality, integrated and person-centred care and support.

  • Mental Health Social Worker or Psychologist or Nurse or Occupational Therapist  
  • Full time or Part time Clinician in Chatswood
  • Multi-disciplinary team in community mental health

The Integrated Primary Care Clinicians role aims to increase access to mental health services in the general practice setting. It involves being an integral part of the general practice team, working directly with individuals and their families to help improve their mental health and wellbeing and providing consultations and advice to other members of the general practice team.

Duties include but are not limited to:

  • Conduct individual sessions and group sessions in the formats of the following: 15 – 30 minute evidence-based interventions are provided for a wide variety of issues (both mental health and physical health related) to people of all ages, with a focus on patient presentation, assessment and need: this may involve prevention, acute care, and chronic disease management.
  • Delivery of high quality and appropriate behavioural interventions relative to the Integrated model of care.
  • Ensure people are supported to identify and achieve results that support improved self-management and skill development.

Essential Criteria:

  1. Registered AHPRA health professional or eligibility for membership of AASW.
  2. Current Unrestricted NSW Drivers Licence
  3. Training and experience in delivering brief intervention such as CBT, MI, and Strengths Based Interventions.
  4. At least 2 years clinical experience working in a mental health setting providing evidence-based psychological brief interventions.
  5. Proven computer skills and knowledge of office equipment.
  6. Experience working with clients who have experienced addiction issues.
  7. Confident in using screening tools.
  8. Experience with working within multidisciplinary teams and ability to work autonomously.
