This well-established non-for-profit organisation is a Brisbane based service supporting needs such as legal, domestic violence, counselling, disability, mental health, employment and various other youth and family based services.

This is a Permanent, Full-Time opportunity, with the option to work a 9 day fortnight. 

On behalf of our client, we are seeking an experienced and effective Case Manager to join a small, supportive Intensive Family Support Team based in Jimboomba (working across Browns Plains / Beaudesert). 

The Intensive Family Support Team is also supported by a Domestic Violence Team and an Integrated Family Housing Specialist. 

The role involves working with vulnerable families to address multiple and/or complex needs and build their capacity to safely care for and protect their children. Engagement with families must involve the active participation of the family in case planning and in demonstrating their willingness for positive change.

The primary focus of the engagement is to strengthen the protective factors within the family to ensure children can safely live at home.

The role involves in home visits, which may be scheduled outside of standard business hours. 

To be considered for this role, candidates must be fully vaccinated for COVID.

 

Key Responsibilities

  • Provide purposeful and tailored case management services to families.  This includes assessment, planning, service delivery in accord with goals, reviewing and record keeping
  • Provide parent coaching support that builds on a family’s aspirations to give their child the best start in life
  • Model and mentor families by providing practical strategies and assistance in such areas as parenting skills, routines, travel, budgeting and tenancy commitments
  • Develop information resources for families and support referrals to other services as required such as medical and mental health professionals, drug and alcohol supports, domestic violence specialist services, child development services, child and youth mental health, education and employment support, and social connections
  • Complete and apply the Queensland Child Safety, Youth and Women’s “Structured Decision Making (SDM) Tools” in line with the SDM policy and procedure manual

 

To Be Successful

  • Minimum Bachelor Degree qualification in Social Work, Psychology, Social Science or similar
  • Demonstrated knowledge, skills and case management experience in working with families, children and young people who are experiencing disadvantage 
  • Demonstrated knowledge of childhood development and how to address family / household concerns
  • Demonstrated experience in applying an appropriate framework for assessment and intervention in relation to family violence and child protection
  • Demonstrated trauma-informed practice in client work and the ability to coach families in strategies to lead them to pathways of independence and participation
  • Effective written communication for case noting, reports etc

Demonstrated knowledge of the Safe and Together Model is highly regarded

 

Additional Requirements

  • Current Queensland C Class drivers license
  • Current First Aid and CPR Certificate, or ability to obtain
  • Current Positive Notice Blue Card
  • Vaccinated against COVID-19

 

Salary and Benefits

  • $89,042 – $93,073 + Super + Salary Sacrifice
