ABOUT THE ROLE
HBNC’s Connecting Communities Social Prescription Program aims to support and empower Fraser Coast residents at risk of social isolation or loneliness, to connect, participate, contribute and thrive in our community.
Based on models developed in the UK and other regions of Australia, social prescription aims to improve health outcomes by addressing the issues of social isolation and loneliness within our community by matching clients with activities, groups or services that support their interests.
The position of Link Worker will work one-on-one with community members, build trust and support them to identify and attend social activities in the community. You will be able to make connections easily and have the ability to connect with local groups and activities in the community. You care about social engagement and have experience working with vulnerable members of the community to help improve their wellbeing.
This position will also help support community groups and clubs to attract and retain new members through training and coaching and will contribute to the collection of resources to support the wider community.
KEY RESPONSIBILITIES
- Work one-on-one with program participants to increase social connection through matching with local activities
- Build trust with participants through listening and interviewing techniques to capture needs and interests
- Be proactive in developing and maintaining links with referrers including medical and allied health professionals
- Develop an understanding of the local community assets including forming connections with local groups, clubs and activities
- Host and attend program events to attract and engage participants
- Administer evaluation and progress tools to capture key information to assist in monitoring the program’s goals.
KEY SELECTION CRITERIA
To be successful in this role, you will need to address in the cover letter:
- Demonstrated experience working one on one with participants to support health, educational, community or wellbeing needs.
- Experience working with vulnerable members of the community
- Experience building and managing a wide range of relationships
- Strong team player with excellent interpersonal skills
- Computer competency including Microsoft Outlook, Excel and Word
- Ability to maintain confidentiality and privacy
- Experience with monitoring and evaluation of programs, services or activities including completing reports
Other requirements:
- Possession of a Diploma or higher in an appropriate field (eg: health, aging, community services, counselling, education, coaching) and experience working with vulnerable people.
- Working with Children Check (Blue Card) and Federal Police Check or ability to obtain. (HBNC can assist to obtain these items)
- Current C class driver’s licence or the ability to gain a driver’s licence within an agreed timeframe.

HBNC is located in the rapidly growing sea change community of Hervey Bay and has become well known as a community meeting place. HBNC was founded 1989 as a community-based initiative to provide help, friendship and support to the most disenfranchised people in the community. The emphasis of all of our activities is on people and the relief of issues and concerns that impede them from living a successful and happy life.
We develop and deliver programs and services in response to community need. Presently, HBNC manages 30 funded programs that encompass aspects of aged care, family services, diversity and social isolation.
This position is located at HBNC’s Urangan Community Wellness Centre.
