Footprints Community is a well-regarded not-for-profit provider of community-based services, working in the community for over 30 years. We specialise in working with older people, those that experience disability, mental illness, as well as those who are at risk of homelessness.
About the Position
Reporting to the Team Leader – Social health Connections Team, this position is responsible for supporting clients who live within the Moreton Bay North sub-region of Caboolture and Kilcoy to address the social barriers that impact on their health and wellbeing, with an aim to reduce experiences of social isolation and loneliness. This position will achieve its objectives in line with the program schedule and model of service delivery set out by Footprints in conjunction with Brisbane North Primary Health Network (PHN).
The Social Health Connection Service aims to make a real difference to outcomes for adults who:
- Are aged 18+
- Living in the region of Caboolture and Kilcoy
- Experiencing social isolation or loneliness
- Are wanting to explore opportunities to improve their social health and wellbeing
About You
- Relevant Certificate IV or Diploma in relevant field (eg. Community Services, Social Work, Human Services) or significant progress towards such a qualification and demonstrated knowledge/experience of the provision of community-based support services.
- Demonstrated experience and understanding of the provision of support services to people who have diverse needs.
- Ability to be respectful, calm, and encouraging towards clients and have patience and understanding of others? journey, including the ability to set boundaries and be self-aware.
- Ability to show initiative, think critically and make judgments based on sound reason and the information/evidence presented.
- Demonstrated capacity to encourage and support individuals to engage in meaningful activities.
- Good written and verbal communication skills, and computer literacy
- Current class C driver license & own roadworthy vehicle, Yellow Card, and satisfactory national police check
- Sound knowledge of local service providers (desirable)
- An understanding of systems including but not limited to: MyAged Care, NDIS, Mental Health Services and Government Agencies e.g., Centrelink, Housing (desirable)
- Current First Aid Certificate (desirable)
