JOB DESCRIPTION

Full Time Contract 

The Role:

We are seeking a Youth Case Manager to work within our Diversion and Prevention Team at Outcare. The successful applicant will provide guidance and support to young people facing criminal charges at Perth Children’s Court. The successful applicant will practice person-centered principles, focusing on the young person’s strengths, and work collaboratively with families and stakeholders.

The Links program supports young people who have unmet mental health issues, psychological needs, and difficulties accessing the supports. The Links team comprises the Department of Health, the Department of Justice, and Outcare – who provide psychosocial supports.

This will involve the following:

  • Work collaboratively with the Department of Health, the Department of Justice and other stakeholders to support young people who have mental health issues and have attracted criminal charges.
  • Participate in joint assessments and use person-centred practice to support young people in the community.
  • Use a client management system to maintain confidential client records.
  • Work towards achieving Key Performance Indicators, which evaluate the program’s effectiveness.
  • Ability to work autonomously and make decisions around complex client problems and communicate effectively to all involved.
  • Participate in multi-disciplinary meetings and negotiate appropriate strategies to meet client needs.
  • Effectively identify client needs and provide support in fulfilling these needs by encouraging participation in community activities and relevant psychological programs.
  • Provide a structured personal support plan that accommodates the needs of the client and is in conjunction with Links.
  • Ensure all services are provided in a culturally appropriate manner whilst adhering to Outcare’s values, processes, policies and procedures.
  • Ensure all services provided comply with the current National Standards for Mental Health
  • Actively coordinate and administer the performance review process within the Program.

 

About you:

  • Tertiary qualification in one or more of the human services fields
  • Mental Health First Aid Certificate
  • Previous experience working with youth with complex needs and their families in a community setting.
  • Knowledge of issues and barriers facing youth with complex needs within the Perth Metropolitan area.
  • Substantial experience in managing complex behaviour and working with clients with mental health issues.
  • High level written, oral and interpersonal communication skills to engage and interact with a wide range of people including clients, internal staff members, government agencies and other stakeholders.
  • Demonstrated ability to plan and prioritise workload to achieve expected outcomes and meet deadlines.
  • Experience in working successfully with clients from disadvantaged, as well as clients from culturally and linguistically diverse backgrounds.
  • Knowledge of the current National Standards for Mental Health Services

 

Other requirements:

Current WA ‘C’ Class Driver’s Licence

Current Working with Children Card

A Nationally Coordinated Criminal History Check, via our preferred provider (a discounted link will be sent to shortlisted candidates)
