An exciting opportunity exists for a Mental Health Clinician to apply for the role of care coordinator, which supports the Northern Beaches Community Mental Health Team to continue to facilitate the delivery of safe and effective health care in a constantly changing and high paced environment.

MHDA Statement of Intention
The intention of the Northern Sydney Local Health District (NSLHD) MHDA is to provide recovery-oriented, trauma informed services that are guided by evidence based practices and collaboration.

As a Care Co-ordinator you will be the key contact to support individuals develop their recovery plan and personal goals using a person-centred and trauma informed approach to care.

The NBCMHS comprises of Community Mental Health Teams each working in a multidisciplinary team comprised of nurses,  psychologists, occupational therapists, social workers and psychiatrists. This ensures consumers and carers are offered a broad range of knowledge and experience in all aspects of adult mental health.

The Northern Beaches Community Mental Health Service (NBCMHS) aims to instil hope, promote well-being and support recovery in the lives of adults living in the Northern Beaches area who are affected by severe and enduring mental illness. We provide an assertive and intensive care coordinated service focused on the individual needs of each consumer. The NBCMHS aims to make a positive difference in the lives of those with whom we work.

Every consumer is allocated a Care Co-ordinator who will be the key contact to support individuals develop their recovery plan and personal goals.

Candidates will need to meet the following criteria:

 

  1. Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
  2. Current AHPRA registration for relevant discipline – Psychology, Occupational Therapist, Registered Nurse, Eligibility for membership with AASW for Social Workers.
  3. Demonstrated, comprehensive mental health assessment and crisis intervention experience. Demonstrated experience and skills in suicide risk assessment and management.
  4. Experience working in community mental health and a demonstrated knowledge of the NSW Mental Health Act.
  5. Extensive case management/care co-ordination skills.
  6. Demonstrated ability to work within a multi-disciplinary team setting.
  7. Proficient in working with data systems and computer skills, in particular knowledge of Microsoft Office Word software.
  8. Current NSW Driver’s licence.
