Step-Up Step-Down (SUSD) Facility
The Mental Health and AOD Service SUSD provides services to adult clients aged 18 to 64 years with a severe mental disorder or illness who require treatment and care in a supportive environment. These clients may require short-term residential support with intensive clinical treatment to prevent them deteriorating and possibly requiring admission to an acute inpatient unit. They may also be clients who no longer require intense clinical treatment in an acute inpatient setting but may still require a short-term continuation of supported 24 hour treatment following discharge from the acute inpatient unit.
Your opportunity
The purpose of this role is to competently deliver discipline specific services, independently and within a multidisciplinary team, to the clients of the Step-Up Step-Down Sub Acute Facility. The purpose of this facility is to prevent an acute admission to our mental health unit (Step Up) and to assist the transition out of hospital (Step Down). You will be a part of a small team of Queensland Health clinicians working closely with non-clinical partners Mind Australia to deliver a psycho-social recovery-based program
