Your new role
Stepped Care for Older Adults is an innovative program for older adults living with mild to moderate mental illness, or experiencing social isolation/loneliness. You will be part of a supportive multidisciplinary team who provide coordinated, integrated, and goal-oriented therapy to older adults. You will provide mental health support to people within the centre and in their own homes.
You will be encouraged to innovate and share ideas through involvement in quality improvement initiatives within the organisation.
You will be responsible for
• Clinical triage including screening of referrals and biopsychosocial assessments
• Goal directed care planning
• Provision of evidence based time limited psychological therapies and/or interventions
• Group development and facilitation
• Care coordination (via akey worker role)
• Supporting consumers activation of choice and control.
What you will need to succeed
The successful applicant will have one of the following professional qualifications:
• Approved Degree in Social Work and eligibility for membership with the Australian Association of Social Workers
• Degree in Clinical Psychology and eligibility for membership with the A.P.S. College of Clinical Psychologists and registration with the Australian Health Practitioner Regulation Agency (AHPRA),
• Approved Degree from a recognised school of Occupational Therapy, eligibility for membership with the Australian Association of Occupational Therapy (Vic.) and registration with AHPRA
• Approved Degree in Psychiatric Nursing or equivalent; or, Bachelor Degree in Nursing and a postgraduate qualification in Psychiatric Nursing, and registration with AHPRA
Demonstrated clinical experience in working with older adults in mental health settings at the level required (e.g. good knowledge of MSE, case formulation, ISBAR handover, as well as ability to demonstrate sound clinical reasoning)
Demonstrated capacity of working in a multidisciplinary team with an ability to build and maintain effective working relationships
A commitment to providing high level of care within a recovery model
Excellent knowledge of frameworks and therapies relevant to their discipline, e.g. family therapy, narrative therapy, and MOHO
