MENTAL HEALTH COMPLEX CASE MANAGER (CLINICAL) – DVA
Work within the Department of Veteran Affairs support service, located in Adelaide but also available in most major cities including VIC, NSW, WA, QLD and Townsville! The aim of the service is to provide specialised, goal-oriented, short term to long-term case management to veterans, defence members and their families identified as having complex and multiple needs. You will play a key role in making a substantial difference in the lives of veterans and their families.
ABOUT THE ROLE:
- Develop risk management plans and strategies to assist clients identified as being at risk and/or with complex multiple needs
- Provide clinical guidance and support to case coordinators who are assisting clients identified as being at risk and/or having complex and multiple needs
- Assist clients to access the required information and support services for their psychological and physical health
- Liaise with the ADF, ex-service organisations, community groups and service providers to facilitate service provision
- Positions available in Melbourne, Sydney, Adelaide, Townsville and Brisbane
ABOUT YOU:
- Preference is for mental health professionals including AASW eligible Social Worker, AHPRA registered Psychologist, Mental Health Registered Nurse
- Allied Health professionals with Mental health case management experience
- Eligible for Working with Children Check & Vulnerable Peoples Check
- Experience coordinating the management of a caseload of complex and potentially at-risk clients in community mental health teams
Salary and Benefits :
- Weekly pay at $60 p/h + Super
