About You:
Highly skilled and motivated individual to work alongside a high-performance multidisciplinary team in partnership with Alfred Mental and Addiction Health to deliver individualised psychosocial recovery orientated care. At South Yarra PARC, you will work with a team of Alfred Mental Health Nurses to provide outstanding mental health care to consumers with complex mental health care needs
You will have:
• Demonstrated flexibility, patience, initiative and emotional resilience
• Capability to advocate for others and build support networks
• Prior experience in mental health support or working with those who are disadvantaged in the community
• Reflective Practice led by a Senior Psychologist
• Access to Alfred Mental & Addictions Health Training Calendar
It is a requirement of this position to be fully vaccinated against COVID-19, acquire and maintain a positive Working With Children’s Check, an NDIS Worker Screening Check and a current & valid driver’s licence.
About the Role:
Work in collaboration with our team of highly skilled Mental Health Workers and Clinical Multidisciplinary Team, including Psychiatrists, a Psychologist, Peer Worker, Social Worker and Nurses. We pride ourselves on a personalised approach and delivering individualised recovery orientated care. The Mental Health Key Support Worker role entails:
• Assessments of individuals with a Mental Health Nurse to determine suitability for PARC
• Deliver evidence-based group content that is curated to the cohort of the ever-changing participant group
• Develop evidence-based care plans that are strengths-based with a holistic wellness focus
• Develop collaborative risk management and mitigation plans
• Enhancing and promoting links with natural supports in the community
• Providing accurate and timely case notes so that colleagues and stakeholders can provide a consistent experience for clients
• Empower people to make connections in the community
• Develop recovery goals, implement steps to achieve set goals
• Monitor and track the progress of goals
• Support planning and review, direct support and exit planning
