ABOUT THE ROLE

Hunter Primary Care (HPC) is offering an exciting opportunity to be part of the team responsible for delivering intake and triage services for primary mental health services across the Hunter and Mid Coast regions of NSW. The successful candidate will apply their strong understanding of the mental health sector across primary and tertiary sectors, as well as NGOs, to process primary mental health referrals and facilitating allocating to appropriate providers. The role will actively participate in activities that inform triage processes, resources and workflows. This position is available on a full time or part time basis, based at HPC in Warabrook (Newcastle) and is currently funded until 30 June 2023.

DUTIES

  • Review and triage incoming referrals for appropriate treatment programs
  • Identify escalation requirements for significant risk and/or complex issues
  • Establish and maintain strong relationships and communication channels with all stakeholders.
  • Liaise with treatment providers to direct ineligible clients to appropriate services
  • Participate in demand management activities.
  • Identify and facilitate performance improvement initiatives

 SKILLS & EXPERIENCE

  • Tertiary qualifications in Mental Health or related field
  • Experience in intake, triage or care planning in a community or mental health service.
  • Strong understanding of the mental health sector across primary and tertiary sectors, as well as non-government organisations.
  • Understanding of stepped care frameworks in a mental health setting.
  • Experience working with/or awareness of the issues impacting on Aboriginal & Torres Strait Islander peoples, and diverse communities, including LGBTIQ and CALD groups advantageous.

Unless a medical contraindication exists, staff at Hunter Primary Care must be fully vaccinated against COVID-19.  Evidence of vaccination status or medical contraindication will be required from the successful candidate.

This position is classified as Health & Wellbeing Services Level 2 per the Hunter Primary Care Agreement 2021. The current remuneration for this classification is $34 to $40 per hour plus entitlements.
