About the service
Dubbo Outreach Service is a free mobile drug and alcohol treatment service for adults that provides individual counselling and case management, and group therapy in the local region. Our aim is to provide a tailored service with a harm reduction focus, that meets the needs of individuals, families and communities experiencing drug and/or alcohol misuse.
Magistrate Early into Treatment program (MERIT) is a voluntary program for adults whom have court matters in the Dubbo or Wellington court whom have problematic alcohol and/or other drug use. MERIT provides access to a wide range of alcohol and other drug treatment services for 12 weeks.
About the role
The role is based with the MERIT Team based at NSW Health. Reporting to the MERIT/Lives Lived Well Team Leader, the MERIT Case Worker will provide assessment, case management support and counselling to eligible client’s for the program with demonstrable drug or alcohol problems whom have been referred to the MERIT program.
Duties and responsibilities include:
- Provide high quality engagement, assessment, counselling and support services to clients, and their families, using evidence based practice principles
- Comprehensive Assessment of clients for entry into the MERIT program
- Complete individual treatment plans
- Updating clinical documentation into Health system
- Manage a case load of clients providing weekly counselling sessions, case management, support, service coordination, referral and advocacy to assist client in achieving their goals. Regularly case review clients with a multi-disciplinary team.
- Assessment of client for entry into residential rehabilitation services or other appropriate treatment options if required.
- Attend regular Team meetings with Lives Lived Well
- Attend mandatory training and identified training
About you
You are an engaging individual with the ability to connect with clients who are navigating difficult life experiences. You can work both autonomously and collaboratively within a team and are able to build effective stakeholder relationships. You will appreciate:
- Tertiary qualifications in Social Work, Psychology, Nursing, Occupational Therapy or similar field
- Understanding of case management practices in drug and alcohol and mental health
- Demonstrated understanding of issues relating to Indigenous people with drug and alcohol substance abuse concerns
- Experience in counselling in a one on one environment and group setting
- Demonstrated solid understanding of Drug and Alcohol treatment issues
- High level of interpersonal, verbal and written communication skills, including report writing
- Capacity to motivate and encourage clients to make change in their lives
- Capacity for Flexibility and self-direction in organising and managing workloads
This is a permanent full time role.. The salary range for this is $74,179.04 to $79,810.64 annually.
