About us

My Home Hospital (MHH) is a new Hospital in the Home Program, funded by Wellbeing SA, and managed by Calvary and Medibank to deliver and grow one of Australia’s largest virtual hospital from home programs. MHH includes a Virtual Care Centre (VCC) responsible for triage, admission, care coordination, medical governance, remote monitoring, and medical record management. Clinical services will be delivered through a network provider.

About this opportunity

As a Care Integration Paramedic, you will collaborate as part of a broader team working closely with Medical Officers, Nurse Care Coordinators, our provider network, and the Local Health Networks (LHNs).

You will be responsible for delivering acute hospital patient-centred care, admit suitable patients for MHH from Local Health Networks, Aged Care Providers and GP practices, coordinate patient care plans, and undertake clinical reviews as part of a multi-disciplinary team.

MHH is committed to providing safe, high quality and effective health care services that address the holistic (physical, social, psychological, and spiritual) needs of our patients and their families and carers. Our design of the MHH model has been informed by core principles that embody the change that we want to see (and create) in acute care.

These positions are based in Adelaide. We can offer part time and full-time positions. Rosters are based over 7 days between the hours of 7am and 8pm.

As an outstanding communicator, your intuitive people skills will be central to your success. You will be naturally empathetic, observant, attentive, trustworthy, clinically sound and committed to making My Home Hospital a centre of excellence.

Working in a dynamic environment applying your clinical skills, you will think on your feet and problem solve to ensure our consumers are provided with best practice care.

You will also have:

  • Bachelor of Paramedic Science (or equivalent)
  • Current unrestricted registration as a Registered Paramedic with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Current Advance Life Support certification
  • Working with Children Check (WWCC)
  • Clinical management experience
  • Understanding of clinical/national compliance standards
