About the program
The Navigator Program supports young people 12-17 years of age who are at risk of disengaging from school to overcome barriers and successfully re-engage with education. The program actively works with young people, their family and support networks to address issues that contribute to their disengagement from education.
About the position
The Navigator Intake and Assessment Worker will also be responsible for ensuring that processes are in place to provide efficient and effective service delivery to young people who are yet to access Navigator Case Management. The Navigator Intake and Assessment Worker is responsible for the contact and support provided to all young people referred to the initial stages of the program, including internal and external referrals, coordination of care, and correspondence with education settings.
