We are currently seeking an Outreach Counsellor to:
- Provide clients with early intervention counselling in individual, family or group settings; practical assistance and support; psychoeducation, information, referral and education according to client needs and contractual obligations;
- Participate in other program activities, such as intake, active hold and client allocation process, as required;
- Undertake assessments and interventions with CYP and their families in the home or at other locations as required;
- Participate in program development and research as required;
- Work collaboratively with other team members and key stakeholders to maintain and enhance service delivery;
- Ensure service delivery adheres to funded activities;
- Demonstrate personal and professional ethics;
- Participate in regular clinical supervision, reflective practice and team case consultation;
- Complete necessary data collection, case notes, client records and other administrative duties;
- Assist in the maintenance of client records to meet privacy, confidentiality and security standards, client file audits and transfer of files;
- Liaise with internal and external professionals to complete assessments or make referrals;
- Contribute to the implementation of systems improvement initiatives;
- Observe protocols to ensure the safety of clients and staff; and
- Maintain professional knowledge and continue to develop knowledge and expertise in relation to professional skills and professional regulatory requirements.
The Role
As a New Horizon’s Outreach Counsellor, you will provide early intervention, counselling support to children and young people (0-18 years) showing early signs, or at risk of developing mental health concerns. New Horizons operates according to the following principles: early intervention, child and young person-centred, family focused, flexible, accessible and responsive. As an Outreach Counsellor you will use attachment-informed, strength and relationship-based approaches to work with CYP and families. You will provide early intervention to address current difficulties and emerging mental health concerns that may compromise the CYP’s developmental outcomes. The program provides a range of services to CYP depending on the assessed need including home or community visiting, counselling, group work, telehealth and referral to other relevant services.
