Hutt St Centre (HSC) is a place of connection and support, empowering people facing homelessness to rebuild their lives, rediscover their identities, and reconnect with their communities.

How you will make a difference
As an Outreach Worker, you will support elderly Aged City Living clients to address the life issues that have caused and perpetuated their homelessness. This will be achieved through housing, social engagement, education, employment, skill development, and improved wellbeing.

Your role will be central in providing practical support to clients, assisting with transportation, home visits, welfare checks, shopping, and goods delivery. You will also provide advice on day-to-day living, including budgeting and personal welfare, and assist clients in addressing complex issues like hoarding to improve their health and wellbeing.

About you
We are seeking applications from experienced case managers or support workers with a deep understanding of confidentiality and information-sharing guidelines. Your ability to engage with elderly clients in a non-judgmental, client-centred way and align with HSC’s values is essential.

This is a full-time, fixed-term role currently contracted until 30 June 2025.

Key to this role is:

  • Proven experience in developing case plans.
  • Strong interpersonal skills to build rapport with diverse, elderly clients.
  • Experience in to educating and assisting elderly clients with life skills.
  • Demonstrated record-keeping skills, including writing case notes and contributing statistical data for research and analysis.

Essentials for the role:

  • Passion for making a positive difference in the lives of those experiencing disadvantage.
  • Strong communication and interpersonal skills, with the ability to advocate effectively.
  • Qualifications in the social services field (Cert IV or above).
  • National Police Clearance and Working with Children Check (or willingness to obtain).
  • A valid full driver’s licence is required due to the mobility aspects of this role.
