An exciting opportunity exists for a health clinician with the Pathway to Community Living Team (PCLI) team in Northern Sydney Local Health District. PCLI is a coordinated state-wide approach to supporting people with enduring and serious mental illness who have been in hospital for more than twelve months and or are likely to experience difficulty with discharge solutions, to re-establish their lives in the community. What you’ll be doing
The Pathway to Community Living Team multidisciplinary clinician will work as part of a multidisciplinary team to provide specialist and innovative advisory and support services to other LHD clinicians and MDT’s for cohorts with high complexity, ensuring a high level of professional practice. The role requires working in close partnership with other stakeholders and supports to create safe, sustainable and person centred solutions for complex and varied clinical issues, challenges and problems across inpatient and community settings.
People of Aboriginal and/or Torres Strait Islander background encouraged to apply.
Candidates will need to meet the following criteria:
- Current AHPRA registration as a Registered Nurse, Occupational Therapist, or eligibility for membership with AASW as a Social Worker in addition to experience in Mental Health.
- Demonstrated skills in the assessment and recovery orientated and person centred care co-ordination of consumers who are seriously affected by mental illness.
- Demonstrated experience to deliver recovery focused interventions for program target group.
- Demonstrated understanding of community mental health services including the NSW Mental Health Act.
- High level written and verbal communication skills & computer literacy.
