JOB DESCRIPTION

About Us?

The Institute for Urban Indigenous Health (IUIH) leads the development, planning and delivery of comprehensive primary health care for Aboriginal and Torres Strait Islander people living in South East Queensland.

The Family Health and Wellbeing Unit, in keeping with the vision of IUIH, aims to support communities in South East Queensland to grow strong and vibrant families, delivering care from preconception, pregnancy and birthing, to early childhood, paediatric and extended family support services.

Birthing in Our Community (BiOC) offers a new model of care that provides comprehensive and culturally informed maternal and infant health services for Aboriginal and Torres Strait Islander families. Supports within the service include access to dedicated family support workers, case-load midwifery care, post-natal clinics, early learning programs and a social health team which includes social workers and psychologists. BiOC is a holistic strengths-based approach to maternal infant health care, operating in a culturally appropriate community-based space.

  • Participate in the provision and review of best clinical practices, including case conferencing with all relevant parties, contribute to client assessment, treatment planning and therapeutic intervention, team meetings, clinical review meetings and use of outcome measures with clients.
  • Identify and support women and families impacted by trauma, while working individually and in groups, to enhance parenting capacity.
  • Case manage complex families through integration and linkages into other appropriate supports and services.
  • Develop and maintain comprehensive client files, which include documentation relating to assessment, treatment planning, therapeutic interventions and outcome measurement.
  • Advocate on behalf of clients.
  • Conduct home visits or community visits, where appropriate.
  • Maintain accurate client records, including the collection and collation of statistical information.
