About the Position
The Practitioner Family Mental Health Support Service will provide intensive, long-term, early intervention support for children, young people, and their families. Practitioners work with the child/young person and their family to identify risk factors and provide education, resources, and referrals to support their wellbeing.
The program also provides psycho-educational and therapeutic group-based programs for children, community outreach, mental health education, and community development activities,
Specifically, the position is responsible for:
- Conduct client assessments and provide case management support
- Provide active and assertive in-home or community-based support to children, young people and their families to support their engagement and access to services
- Collaboratively develop and implement with families the Family Action Plan. Provide tailored and appropriate responses to children, young people, and families.
- Facilitating psycho-educational group-based programs for children, parents, and carers.
To be considered for this position, you will require:
- Tertiary qualification in Social Work, Community Services, Education, Youth Work, Counselling, or relevant experience with a focus on mental health and working with children and young people in context.
- Demonstrated skills in developing and delivering pyscho-educational group programs with strong engaging and confident group facilitation skills
- Demonstrated ability to quickly build rapport and provide a psychological safe place for sharing with students.
- Demonstrated ability to deliver case management services to families with mental health issues
