JOB DESCRIPTION

Exciting Opportunity in Gympie for our next Case Manager/Practitioner

Join our dynamic team in Gympie. We are currently seeking a dedicated Practitioner/Case Manager to join us on a full-time fixed term contract for twelve months. In this role, you will be at the forefront of providing direct therapeutic interventions to children and young people while offering crucial support to foster and kinship carers.

Key Responsibilities:

  • Establish meaningful connections with children and young people across various age groups, utilising a micro-therapeutic approach to enhance resilience and understanding of personal history.
  • Assess strengths and risks within care placements, ensuring Foster and Kinship Carers acquire and maintain the knowledge, skills, and capability to provide safe and supportive care.
  • Collaborate with key stakeholders to develop and implement targeted, time-limited therapeutic interventions for children, young people, and carers, tailored to different developmental stages (0-17 years).

Qualifications and Experience:

  • Tertiary qualification in Social Work, Psychology, Social or Behavioural Sciences, or a related field.
  • Previous experience in working with carers, including training, recruitment, and onboarding.
  • Proven experience in assessing complex needs, developing individualized care plans, and providing in-home support.
  • Knowledge and understanding of child protection, including the Children Protection Act.
  • Demonstrated expertise in training and facilitating support groups.
  • Ability to operate within child-centred frameworks within a family-focused network.
  • Timely completion of complex documentation within required guidelines.
  • Current Blue Card (Working with Children) or willingness to obtain.
  • Current Licensed Care Service (LCS).
  • Current Queensland Driver’s License.
  • Current COVID-19 Vaccination Certificate.
