JOB DESCRIPTION

We are a voice for social change in regional Australia, with a history spanning over 47 years. Your role within our Family Connect in Schools Program in the Port Macquarie region, will allow you to:

  • Create and deliver innovative high quality, strengths-based support driven case management interventions, solutions, and assessments.
  • Provide practical support and assistance to families, children, and young people.
  • Assist families navigate and connect with appropriate support and referral services.

Utilise your knowledge to achieve optimum outcomes.

Joining us will give you the opportunity to work with a team of dedicated individuals supporting children, families, and young people within schools. Your day-to-day role will include intake assessment, coordinating appropriate referrals and providing assistance to families, children and young people. Your work will be diverse and impactful.

We are looking for an individual with a desire to make a difference. The successful candidate will have:

  • Tertiary qualifications or equivalent experience in education, health, psychology, social work, social science or social welfare, health promotion or similar
  • Experience providing high quality Intake, Assessment and Referrals for families, children, and young people in a school environment.
  • Experience providing high quality Case Management support to families experiencing a myriad of challenges, underpinned by a child focused safety lens.
