At Grace Recovery Hub, we believe in empowering people with psychosocial disabilities to navigate their unique journeys. As a Psychosocial Recovery Coach, you’ll work closely with individuals to help them understand their National Disability Insurance Scheme (NDIS) funding and options, while supporting them in setting and achieving their personal goals.
We are seeking a passionate Psychosocial Recovery Coach to support NDIS participants in building resilience, self-sufficiency, and personal capacity. Working closely with participants, you will help them create and implement individualised recovery plans, connect them with community and mainstream services (such as housing, health, and education), and coordinate ongoing support. This position begins with 15–20 hours per week, with the potential to increase as you establish yourself in the role. Office attendance is required one day per week for supervision and team meetings.

You may have lived experience or a background in fields such as mental health, case management, support coordination, or care coordination. Qualifications in health, social work, disability, mental health, community services, or alcohol and other drugs are highly valued. Most importantly, we’re looking for people who share our dedication to supporting individuals with psychosocial disabilities.
Key Responsibilities
- Collaborate with participants to build and implement individualized recovery plans focused on strengths, goals, and resilience.
- Prepare NDIS reports, assist participants in plan reviews, and ensure that minimum billable hours are met.
- Support participants in coordinating services and selecting providers, empowering them to navigate their recovery journey independently.
- Conduct assessments aligned with support plans and maintain clear, concise case notes.
- Manage an individual caseload and engage with key stakeholders to enhance each participant’s experience.
Qualifications & Experience
- Diploma in Mental Health or Bachelor’s in Psychology or Social Work (highly desirable).
- Minimum of 1 year of experience in the mental health or related field.
- Experience in Support Coordination is highly desirable.
- Full Australian Driver’s License, First Aid/CPR Certificate, NDIS Worker Screening Clearance, Working with Children Check, and NDIS Worker Orientation Module certificate (or willingness to obtain these).
