360 Health + Community is a person-centred health service committed to facilitating excellence in integrated, holistic healthcare for all community members.  Our local teams are based in Perth, Peel, Geraldton and Kalgoorlie to bring care and support to people when, where and how they need it most through well known, trusted and successful programs such as the Street Doctor, headspace, ALIVE, Regional Mental Health Care, Healthy Lifestyle Supports, Psychosocial Supports, and NDIS Therapy, Recovery Coaching and Support Coordination.

 

The Psychosocial Support Program supports people who have a mental illness which impacts on their functional capacity. These supports are non-clinical, community-based and focus on capacity building and enhancing stability in their day to day living.  Our experienced team assists clients with mental health challenges living in the community and who may have coexisting disabilities and/or complex needs.   Through compassion, collaboration and integrity we work closely with clients, their families & carers towards capacity building in the areas of community access, inclusion and day to day living. 

 

Psychosocial Support Workers are responsible for the assessment of clients to develop their Recovery Action Plans and, where a client wishes to test their eligibility for the NDIS and there is a prima facie case, to assist and guide the client in making an application.

 

We are seeking applicants who:

• Have experience in a mental health support worker role.

• Are experienced in mental health and/or disability services.

• Understand recovery oriented and client centred practice.

• Keep excellent case notes (we use Lumary).

• Enjoy being part of a larger team and building relationships with internal stakeholders, therapy partners and community support organisations.

• Hold a minimum Certificate IV qualification in Mental Health, Community/Disability or equivalent.

  • Hourly rate between $35.42 and $39.63 depending on skills and experience (annual full time equivalent $69,989 to $78,308 per annum)
