The position:

This position forms part of our Individualised Services team that specialise in supporting people with psychosocial disability. The Support Worker position focuses on assisting participants to meet the goals within their individualised NDIS plan. The NDIS aims to provide people with choice and control over how, when and where those supports are provided. This role aims to promote independence and confidence in our participants, to enable them to live their best lives. This is an outreach community based role, providing supports in participants homes and providing transport and support in accessing the wider community.

Key Responsibilities (not limited to):

  • Establish supportive, engaging and professional relationships with participants, to build their capacity to work towards achieving their individual goals and outcomes outlined within their NDIS Plan
  • Utilise recovery oriented principles to provide a high standard of direct support in areas including but not limited to independent living skills, social and community participation, personal choice and decision making, whilst building upon a participants strengths, increasing resilience and improving mental health.
  • Maintain accurate and professional service delivery notes and documentation.
  • Work both autonomously in a community outreach environment, and collaboratively within a team environment to deliver high quality psychosocial supports.

About You:

This is an opportunity for an experienced Support Worker with a genuine interest in providing excellent service to our clients.

  • You have demonstrated the ability to establish supportive, engaging and professional relationships with participants; to build their capacity to work towards achieving their individual goals and outcomes outlined within their NDIS Plan
  • You have utilised recovery-oriented principles to provide a high standard of direct support in areas including but not limited to independent living skills, social and community participation, personal choice and decision making, whilst building upon a participants strengths, increasing resilience and improving mental health.
  • You have maintained accurate and professional service delivery notes and documentation.
  • You can both autonomously in a community outreach environment, and collaboratively within a team environment to deliver high quality psychosocial supports.
  • And finally, you’re someone who’s passionate with a demonstrated capacity to promote and uphold values including discretion and confidentiality.

Minimum Qualifications:

  • NDIS Worker Screen Clearance,
  • Proof of Australian Working Rights,
  • Working With Children Check,
  • Current Drivers License & Motor Vehicle Insurance,
  • First Aid Certificate
  • Certificate IV in Mental Health/Community Services
