JOB DESCRIPTION

  • Fixed Term Full Time contract until June 2024
  • Brisbane South (PAH, Logan & Bayside Hospital Service Area)

Footprints Community is a well-regarded not-for-profit provider of community-based services, working in the community for over 30 years. We specialise in working with older people, those that experience disability, mental illness, as well as those who are at risk of homelessness.

About the role & Program

The Recovery and Wellness Program (RWP) is a psychosocial outreach support program for people who have a severe and complex mental illness and reside in boarding houses, supported accommodations and crisis accommodation including (couch surfing). The Recovery facilitator provides one to one support to assist the program participant to design and work towards achieving their goals through engaging in recovery-oriented practice framework. The RWP works in partnership with the Metro North and Metro South Hospital and Health Services. All program participants are referred by clinicians through Queensland Health?s Mental Health Services.

The Recovery, Wellness and Housing Service aims to make a real difference to outcomes for those who meet the requirements:

  • Individuals aged 18 years and over
  • Experiencing a severe mental illness
  • Accessing or recently (within the last three months) accessed mental health clinical care through a Hospital and Health Service (HHS)
  • Referred by the HHS
  • Residing in boarding houses, crisis accommodation or hostel

About you

  • Prior experience in a similar role, with knowledge of hoarding needs and the principles of decluttering, or the capacity to quickly learn.
  • Ability to be respectful, calm, and encouraging towards consumers and have patience and understanding of others? recovery journey, including the ability to set boundaries and be self-aware.
  • Ability to show initiative, think critically and make judgments based on sound reason and the information/evidence presented.
  • Demonstrated capacity to encourage and support individuals to engage in meaningful activities.
  • Well-developed interpersonal, verbal, and written communication skills, and the demonstrated ability to work effectively in a team environment.
  • Computer literacy and the ability to effectively manage a client data base.
  • Ability to work remotely from a team when required, and ability to utilise virtual meeting platforms e.g., Microsoft Teams, Zoom.
  • Current class C drivers? licence.
  • Ability to use your own insured vehicle.
  • National Workers Screening check or current Yellow Card.
  • Covid-19 Vaccination requirements as determined under the Public Health Directions and/or alternative measures as determined by Footprints
  • Current First Aid Certificate and CPR
  • Relevant qualification in Community/Human Services (advantageous)
