Service Specialist – (After Hours/Weekend) Primary & Community Care Services

JOB DESCRIPTION

SUMMARY

Primary & Community Care Services (PCCS) coordinates a community-based after hours drop-in center to provide a safe place and engaging service to our clients.

As a service specialist you will be providing after-hours/weekend support to the clients living with long term health conditions, including mental illness, and complex needs. You will also be required to assist with general reception coverage and various administrative tasks arising from the client services operations.

WHAT YOU’LL DO

  • Support clients with mental health and wellbeing through fostering social inclusion, connectedness, and equipping them with information.
  • Establishing a compassionate environment by providing emotional and psychological support to the clients.
  • Identifying and assessing any care requirements for individual clients
  • Performing risk assessment and referral to suitable services 
  • Assist with general reception duties, facilitate efficient administration and record keeping.

WHAT TO BRING

  • Current AHPRA/AASW Registration
  • A strong knowledge of disability & mental health services.
  • Ability to deal with emotionally charged and pressured situations.
  • Understanding of risk management, identifying emerging risks and ensuring risk mitigation action plans are developed and followed through
  • Knowledge and experience working cross culturally
  • Relevant vaccination status 

**Suitable for Enrolled Nurses, Registered Nurses, Social Worker and all Allied Health professionals
