About the Role
- To provide holistic case management service under our settlement programs (HSP & SETS) to newly arrived refugees and humanitarian entrants (Clients), including those with complex needs requiring specialised and intensive services, to build the skills and knowledge for social and economic well-being.
- The Case Worker will be the central point of contact and has a responsibility for the management of the delivery of Settlement Services to Clients, delivering Settlement Services to clients on a needs-based case management approach.
- The role is to case manage working in partnership with Clients and other service providers, to achieve sustainable outcomes.
- Must understand the level of need and the complexities of circumstances.
YOUR responsibilities:
- Working autonomously deliver Settlement Services to Clients as per MCCGC policy and procedures and HSP & SETS Guidelines
- Manage and coordinate deliverables, foundation services (including English, Education, and Employment), housing and components to settlement clients through case management
- Review and update pre-arrival need assessment based on information from the assessment systems and or links
- Develop knowledge and understanding of the wider program requirements, ensuring compliance expectations
- Ensure data accuracy and administrative management to support relevant outcomes in a timely manner as per the contractual requirements
- Development of networks and ongoing liaison with key stakeholders to meet client needs
About YOU:
- Minimum 3 years equivalent experience and/or Tertiary qualifications in Human Services, Social Work, or relevant Discipline
- Trauma-informed approach with case management experience with a good understanding of client-centred, holistic case management practice framework; and the ability to provide provision of advice and referral pathways as relevant.
- Demonstrated experience in case management supporting newly arrived refugees and humanitarian entrants during their initial settlement in Australia
- Demonstrated understanding of support needs of clients going through crises (e.g. family violence, homelessness, child protection, acute mental illness, etc.)
- Strong background in conducting needs assessments with clients
- Extensive experience developing, implementing, and reviewing case plans
- Demonstrated experience meeting Key Performance Indicators
- Demonstrated ability to adhere to quality standards and compliance requirements
- Must hold a current Driver’s License.
- Hold a current National Police Certificate or has the ability to obtain one.
- Must have knowledge/experience working with a culturally and linguistically diverse workforce
- Must be fully vaccinated and be able to present vaccination record
- Must complete the Department of Health COVID-19 Training Modules
