What You Will Be Doing
The Social Support Program runs from Hastings and Mornington Community Health. The program offers an active group for younger (under 60 years) people with a disability such as an acquired brain injury/intellectual disability and the other groups are for older adults with various physical disabilities, dementia, social isolation and carer support needs.
The program aims to maintain skills, provide meaningful activities including outings that enhance wellness and reablement while linking clients and carers to health services as required. The Social Support Group Worker aims to assist participants of the program to engage in social activities whilst endeavoring to maintain skill retention and development.
Key responsibilities include:
• In conjunction with the Team Leader and other workers, clients and volunteers develop and deliver innovative, skill-focused activities suitable to the needs of the client group.
• Contribute positively to a culture of participant focused service delivery.
• Assist participants with personal care as required.
• Assist in providing transport for participants as required including driving the 12 seater bus
• Assist participants with activities as required.
