The Role:
An exciting opportunity exists for an enthusiastic, dynamic and innovative Grade 2 Social Worker to join the Acute Social Work team at Northern Health Epping.
This role requires a Grade 2 Social Worker to provide comprehensive psychosocial assessment, interventions and discharge planning to patients and their families or carers attending the Paediatric Outpatients clinic at our Epping site. This is a 2 days per week position, days of work flexible.
To be successful in this role you will have:
- Highly developed interpersonal, practical negotiation, conflict resolution and communication skills.
- Excellent organization skills, yet be flexible, to enable you to meet tight deadlines.
- Excellent and concise documentation skills.
- A demonstrated capacity to work as a team player both within the multi-disciplinary team and the Acute Social Work team and be an excellent communicator who is comfortable engaging productively with staff at all levels of the health service.
- Significant experience in undertaking comprehensive psychosocial assessments, intervention, discharge planning of patients in an outpatient setting.
- A commitment to self-initiated continuing professional development.
- A thorough knowledge and experience of working with children and their parents/carers, Child Protection Services, family violence survivors and services and patients with mental health concerns.
