About the role
South West Healthcare (SWH) is seeking a motivated and skilled person to join the Aged Persons’ Mental Health Service (APMHS). As a valued member of our multidisciplinary team, you will provide a service which includes direct clinical treatment to consumers, families and carers including short and long term case management. The role includes assessment and evidence based treatment interventions of a high standard as well as delivery of outreach support to consumers in their own environment, residential care facilities and in other settings as indicated by clinical guidelines.
Our Social Workers work using systematic mechanisms that ensure regular communication and the flow of information with other clinical staff and professionals, families, carers and stakeholder groups located both internally and externally to SWH.
This is a permanent full-time position (80 hours per fortnight) with a monthly ADO with an immediate commencement date.
This position is eligible for an incentive payment for rent, childcare, relocation costs and more, as part of the new workforce and incentive grants program administered by Rural Agency Workforce Victoria (RWAV).
What you bring
The successful applicant will be a qualified Social Worker with the ability to demonstrate excellent skills and broad experience in mental health clinical services, including a proven ability to work within a team based multi-disciplinary model. Previous experience in working with elderly consumers experiencing mental health difficulties or ongoing case management would be an advantage.
Working as part of a collaborative and highly skilled team of professionals, excellent verbal and written communication skills are also essential. A comprehensive understanding and knowledge of legislation, regulation and contemporary practice is a must.
Applicants must hold a tertiary degree in Social Work which allows for eligibility for membership with the AASW.
