About the role:
As the Grade 2 Social Worker you will be responsible for providing clinical social work services to referred patients and families. A regular day will see you undertake assessments, provide numerous social work interventions (including counselling) that support patient recovery and assist patients to discharge from hospital. You will be a member of a multi-disciplinary team planning care and attend case conferences. You will also undertake other non clinical activities such as quality improvement work, training and supervision.
Key responsibilities will see you:
- apply your social work knowledge and skills to assess, treat and plan the discharge of patients
- provide counselling to patients and families
- supervise grade 1 social workers
- supervise and train social work students
- participate in quality improvement, or research
You must have:
- a degree in Social Work
- ideally have worked in a health related setting
- ability to work and contribute to team culture and work within a multidisciplinary environment
