What you will be doing

  • You will conduct health assessments to evaluate client’s needs and create plans, recommendations and take corrective actions
  • You will work collaboratively as part of the multidisciplinary team to provide holistic care to the client and liaising with their carers, case managers & other health partners
  • You will have the opportunity to contribute to our Wellness for Independence group programs and promote health and wellbeing in the community
  • You will be in a rewarding role where you can make a big difference in someone’s life and help keep people independent in their own home to live life to the fullest
  • You will benefit from a flexible work environment – work from home and plan your day including seeing clients 

What you will bring

  • Bachelor Degree or higher in Social work and AASW membership and accreditation.
  • Satisfactory Police check
  • Current First Aid and CPR
  • Current driver license and ability to travel for client care
  • Highly effective communication and interpersonal skills
  • Well-developed IT skills and experience with client management software
  • Experience working with a multidisciplinary team and exposure to collaborative practices
  • Sound clinical knowledge and ability to deal with complex issues
