What you will be doing
- You will conduct health assessments to evaluate client’s needs and create plans, recommendations and take corrective actions
- You will work collaboratively as part of the multidisciplinary team to provide holistic care to the client and liaising with their carers, case managers & other health partners
- You will have the opportunity to contribute to our Wellness for Independence group programs and promote health and wellbeing in the community
- You will be in a rewarding role where you can make a big difference in someone’s life and help keep people independent in their own home to live life to the fullest

What you will bring
- Bachelor Degree or higher in Social work and AASW membership and accreditation.
- Satisfactory Police check
- Current First Aid and CPR
- Current driver license and ability to travel for client care
- Highly effective communication and interpersonal skills
- Well-developed IT skills and experience with client management software
- Experience working with a multidisciplinary team and exposure to collaborative practices
- Sound clinical knowledge and ability to deal with complex issues
