The Role:
The key purpose of this role is to link people with health and social wellbeing needs, non-clinical sources and support within their local community. This includes maintaining a centralised system of resources, ensuring that the service entries are up to date and available for staff. You will also be responsible to establish local networks with service providers and organisations and facilitate the exchange of information. You will also be responsible for connecting people into community, and improving the ability of community members to access services and supports. There’s no case management here. Just working with people, creating opportunities. People manage their own destinies, we just provide the chance to consider, reflect, identify options, and make meaningful choices. If you’re creative and socially engaged, you want to be out in community, creating opportunities for people to support each other, this role is for you!
Essential Criteria:
- Experience in working within disability sector
- Degree in Social Work
- WWCC and Police Check
- Driver’s License and Comprehensive Car Insurance Policy
