A little about the role

The position sits within the Mental Health, Family Violence & Community Outreach Directorate.

The role has the overall responsibility for: Working within a team to ensure evidence-based practice is provided to clients and babies. 

The role involves providing assessment, care coordination, education to clients, and supporting self-management approaches to clients with a range of presentations during pregnancy and in the immediate post birth period.  The role also assists with onward referral and identification of equipment needs.   

About you

  • You have successfully completed a Degree of Social Work and are eligible for membership with the Australian Association of Social Workers.
  • You have a minimum of 2 years practice.
  • You have demonstrated experience in working with vulnerable clients from diverse backgrounds in a service delivering perinatal care, or in a community setting.
  • You have well-developed clinical skills and demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes as agreed with client, families, and other stakeholders.
  • You have knowledge and experience in the areas of the prevention of family violence, and in child protection.

The fine print

  • Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), an international residency statutory declaration and hold a valid employment WWCC and NDIS Clearance Check.  
  • Full driver licence and travel between sites required.
  • Must be fully vaccinated (COVID-19) and adhere to the Public Health Order regarding vaccination in healthcare facilities.
