Eunoia Lane is a highly regarded, leading provider of NDIS Support Coordination and Occupational Therapy services across Northern Australia.

Eunoia Lane currently provide Support Coordination, Specialist Support Coordination, Case Management and Occupational Therapy services to Territorians living with disability or mental health diagnoses.

We are currently seeking a pro-active Social Worker/Specialist Support Coordinator to join our enthusiastic team.

The Role

The position will be working with clients with complex care and medical needs across the life span, to identify and address barriers to implement their NDIS plan. The Social Worker/Specialist Support Coordinator is required to build relationships with clients, their family, disability services organisations, clinical staff and various government departments, including health, education, communities and justice.

Eunoia Lane encourages a collaborative approach by stakeholders to ensure supports are delivered in a person-centred and timely manner. This position assists with building a foundation for best practice care, staff training, support and intervention for clients, ultimately achieving positive and sustainable outcomes following a period of crisis.

Key Requirements

  • A bachelor’s degree in social work.
  • Demonstrated well-developed oral, written and interpersonal communication skills.
  • Demonstrated ability to work well independently and cooperatively as a member of a team.
  • Established ability to listen actively and demonstrate empathy.
  • Well-developed organisational skills, the ability to manage time effectively and prioritise tasks.
  • Working with Children (Ochre) card; Police check (within 6 months); current Drivers Licence and roadworthy vehicle.
  • Computer literacy – efficiency in Microsoft Office.

Desirable

  • Experience working with clients with dual diagnoses. Experience in Forensic Disability or Forensic Mental Health is desirable but not essential.
  • A willingness and ability to travel by car, 4WD, and/or light aircraft to rural and remote settings.
  • Experience working with Aboriginal and Torres Strait Islander peoples with an interest in indigenous advancement.
  • Experience working with Culturally and Linguistically Diverse (CALD) people and their support networks.
