Weekend Social Worker, Grade 2, 16 hours (Sat and Sun)

About the role:
As the Weekend Social Worker, you will be responsible for providing social work services to referred patients and families across the following areas: Intensive Care Unit, Emergency Department and General Medicine (primary location).
Key responsibilities will see you:
– undertake assessment practice, social work interventions and planning for discharge for referred patients.
– facilitate family meetings
– undertake family violence and vulnerable children risk assessment and response
– provide counselling services to patients and families
You must have:
– A degree in Social Work
Ideally you have:
– previously worked in a hospital setting
– an ability to work autonomously, in a multi-disciplinary environment
