Social Worker – Grade 2
Aged Psychiatric Assessment / Treatment Team (APATT)
- Full Time position working 80 hours per fortnight – 8.30am-5pm
- Excellent work-life balance; Monday-Friday roster plus an accrued day off each month
- Great location based at Royal Park Campus in Parkville, close to public transport and onsite parking
- Work within Public Community Mental Health as part of Royal Melbourne Hospital – follow an outreach model servicing Older People within their home.

The Role:
- Case-management guided by recovery and biopsychosocial models
- Holding a caseload of consumers with complex and severe mental health related needs
- Engage in new assessments and regular reviews with consumers
- Participation in duty roster – managing in-coming community referrals and inpatient bed requests
- Prioritizing and attending to crisis as required
- Communication with family, significant others and linking consumers with external organisations
About You:
- Demonstrate a good understanding of mental health needs for older adults (primarily 65years +)
- Negotiate and collaborate effectively at all levels
- Work well autonomously and in larger team environment
- Previous experience working as a mental health clinician in either a community or inpatient setting
- Proven ability to undertake a range of biopsychosocial interventions with consumers
- Passionate about consumer care
