About the role
This is an exciting opportunity for a senior Social Worker to join our Hospital Admissions Risk Program(HARP) at Eastern Health. The HARP social worker will be supporting a complex cohort of clients in the community who have chronic diseases and psychosocial challenges. The HARP Social Worker utilises their knowledge, skills and clinical reasoning to conduct psychosocial assessments including risk assessments, as well as social work interventions for multiple complex issues. The HARP social worker requires a sound understanding of health service systems and provides secondary consultation to colleagues within the multi-disciplinary program in order to support specialised HARP team processes, working together to deliver best health care experiences and outcomes.

About you
To be successful in this role you will possess:
- Hold a degree qualification in Social Work that has been approved by the Australian Association of Social Workers (AASW) for membership as a social worker.
- Maintains compliance with the AASW Continuing Professional Development Policy in order to meet the Eastern Health Allied Health credentialing standard.
- Demonstrates an advanced knowledge base specific to medical diagnosis and related social health issues presenting in an Ambulatory setting and demonstrates the ability to undertake risk management across the continuum of care.
- Excellent interpersonal skills including demonstrated flexibility, proficient negotiation and communication skills (written and verbal).
- Capacity to work skilfully and creatively in an environment of competing values.
- Demonstrate a proactive approach and change management skills.
- Strong conceptual skills to provide high-level advice to the Allied Health Manager, Social Work Clinical lead and Program Manager on program, practice and policy matters.
- Computer literacy.
- A current Victorian driver’s licence.
