JOB DESCRIPTION

  • $41.95-$52.95 per hour* depending on experience

Are you hoping to join a team of multi-disciplinary professionals and support staff that are truly making a difference to the lives of older people? We are currently seeking to appoint a suitably experienced Physiotherapist to join the allied health team at our Transition Care Program at our Juniper Korumup aged-care facility in Albany.

Purpose of this Position:

The Social Worker position provides social work services to the clients, their families and staff of Juniper incorporating the wellness approach. Care and services are to be responsive, timely and empowering, enabling the client to lead a continued safe and independent value added life within their own home.

Key Result Areas

  • In conjunction with the residential/community standards:
  • Provide a social work service to clients and their families of Juniper Charles Jenkins.
  • Administration and Professional Accountability.
  • Compliant with Occupational, Safety and Health requirements including appropriate Injury Risk

The Skills, Knowledge and Experience required:

Qualifications

  • Tertiary qualifications in social work or human services area.
  • Eligible to join Australian Association of Social Workers (AASW).
  • Significant experience in social work within an aged care setting.

Knowledge 

  • Knowledge of aged care within a community/residential setting. 
  • Knowledge of and commitment to occupational safety and health.

Experience 

  • Experience in provision of social work services within an aged care setting.
  • Experience in monitoring budgets and expenditure.

Skills

  • Client assessment skills. 
  • High level written and verbal communication skills.
  • Problem solving skills.
  • Able to initiate and implement continuous improvement strategies.
  • Excellent teamwork and liaison skills.
  • Computer literacy (desirable to have worked with a client management database).
  • Time management skills.
  • Ability to work without direct supervision.
