What you will be doing:

The Social Worker accepts referrals from medical, nursing and allied health staff and from patients and their families and initiates referrals where appropriate. This role utilises a variety of social work methods, the social worker provides assistance to patients and their families who are experiencing emotional or social problems precipitated by, or contributing to their illness, disability, the crisis of hospitalisation or changes to their life situation.

In this role you will be responsible for; psychosocial assessment of patients and their families, consultation with other members of the health care team, participates in the development of an appropriate management and discharge plan for patients and providing ongoing social work service to the patient and family appropriate to their identified social needs. These may include crisis counselling, counselling, grief and bereavement counselling, co-ordination of social welfare and community resources, arranging alternative accommodation, arranging and/or providing follow up services after discharge, and participating in family conferences, interviews, case discussion and team meetings with other members of the health care team.

Selection criteria to be addressed:

  1. Tertiary Qualifications in Social work and Eligibility for the Australian association of Social Workers (AASW)
  2. Previous social work experience in a hospital setting is preferred
  3. Experience in working with clients with chronic and complex needs resulting from disability and illness
  4. Experience working with aged care clients , including applications for Guardianship
  5. Experience in providing psychosocial assessment, crisis counselling, grief and loss counselling and conflict resolution
  6. Demonstrated ability to provide high quality professional assessment and care plans within a fast paced setting where there are competing demands
  7. Demonstrated high level communication skills, (written and oral) negotiation advocacy and demonstrated computer skills
  8. Current Drivers Licence, including P1 or above, valid within Australia
