About the Opportunity
The Social Work Department is currently seeking an enthusiastic team member to join our small team in the Southern Highlands. 

This role is unique as it is based within the hospital and provides social work interventions as part of a multidisciplinary Specialist Aged Care Team (SpACT) for aged people living in the community. This role provides an opportunity for home visiting along with office-based assessments, alongside the Occupational Therapist and Physiotherapist on the SpACT team, you will provide the care co-ordination of services and supports to ensure the emotional, psychological and social needs of the aged residents within the community are met. 

What You’ll be Doing
To provide Social Work services to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.  

As a member of the multidisciplinary Specialist Aged Care Team (SpACT) conduct comprehensive psycho-social assessments and provide discipline specific interventions and care co-ordination service for frail elderly people living in the community.

Respond to the emotional, Psychological and soial needs of frail aged residents referred to the SpACT program.

Respond to referrals from the Emergency Department of Bowral Hospital for aged care clients who require timely assessment and support which could prevent an admission to Hospital.

How to Apply
To be considered for this position, please ensure you address the selection criteria as thoroughly as possible.

  1. A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW). 
  2. Demonstrated skills in the comprehensive psycho-social assessment of older people and adults with complex co-morbidities 
  3. Demonstrated relevant clinical experience working with frail, aged clients and their families and knowledge of relevant community services including the Guardianship Tribunal. 
  4. Demonstrated experience prioritising work and managing stressful situations
  5. Proven ability to work independently and collaboratively within a multi-disciplinary team.
  6. Demonstrated high level communication skills (written and verbal) and computer skills
  7. Evidence of commitment to clinical supervision and professional learning
  8. Demonstrated understanding of, and commitment to quality processes, and the application of evidenced based practice within social work.
