Role:
Mates4Mates is currently seeking an experienced Social Worker to join our Mates4Mates Veteran & Family Wellbeing Centre. The purpose of this role will be to work collaboratively as part of a multidisciplinary clinical team to support the sustainable delivery of services to our members in addressing psychological, emotional and behavioural issues.
In this full-time position you will be delivering care coordination, case management, individual counselling, group programs and supporting social connection activities. You’ll be working with a progressive and motivated team delivering evidence-based treatments – you’ll find that no two days are the same.
Your responsibilities will include:
- Conduct intake activities for clients in accordance with Mates4Mates policies and procedures. Activities may include, but are not limited to eligibility assessments, intake interviews and psychosocial assessments.
- Provide coordination of care and case management of veterans and families with complex care needs, transition care needs and care integration needs.
- Take a lead role in the coordination of care for our clients through case conferencing, liaison and communication with internal and external stakeholders.
- Deliver individual and group mental health support and or interventions to veterans and family members, utilising billable opportunities where possible in line with DVA, Medicare and other referral policies and procedures.
- Provide mental health triage and referral support to Liaison Officers and Regional Manager.
- Develop and deliver health and wellbeing projects and programs as directed by the Clinical Program Manager and Clinical Lead – Social Work and Counselling.
- Provide support to veterans and their families through participation in Veteran and Family Wellbeing Centre activities, including social connection activities.
Essential for success:
- Relevant qualification in social work.
- Registration with an external certifying body such as the Australian Association of Social Workers.
- Experienced in working directly with community members experiencing a range of social, financial, physical and/or psychological issues.
- Experienced in intake and assessment or conducting structured interviews and assessments with people seeking assistance within a community services environment.
- Well-developed oral and written communication skills, interpersonal skills and computer proficiency, including the ability to use client information management systems.
- Demonstrated accountability and proficiency in record keeping.

- Direct experience in delivering services to current or ex-serving Defence personnel and their families.
- Experience working in a multidisciplinary team.
