An exciting opportunity exists for a Social Worker with a strong interest in mental health rehabilitation and Homelessness to join an intensive rehabilitation team. The Mental Health Consumer Homelessness Intensive rehabilitation team provides a novel approach to managing the challenges experienced by Mental Health Consumers dealing with Homelessness.

What you’ll be doing

Working as part of a multidisciplinary mental health team offering an intensive rehabilitation programme to identified consumers with the goal of empowering the consumer on their rehabilitation pathway. This will involve robust assessment of current functioning and mental and physical health status and working with the team to maintain, and where possible, improve  current interpersonal and living skills, whilst support the consumer’s journey back to living in the community.

Candidates will need to meet the following criteria:       

  1. Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
  2. Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.
  3. Proven ability to work independently as a mental health clinician and to establish effectively collaboratively working relationships within a multi-disciplinary team and interagency networks.
  4. Demonstrated well developed interpersonal, written and verbal communication skills with the ability to communicate with internal and external stakeholders.
  5. Demonstrate skills and knowledge in the provision of service within a mental health setting to consumers, carers and families requiring assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral and discharge planning.
