JOB DESCRIPTION

Mercy Community Aged Care Rockhampton supports and inspires people in need to live healthy lives within inclusive communities. With both residential facilities and community programs, Mercy offers a range of services and believe a big part of what we do is to help our residents and clients flourish, knowing what is important to them and their individual needs. With a focus on person-centred care, you will support the team to provide a high level of quality care to the people we support.

 

Your Role:

Working with clients in the community, you will:

  • Deliver sound social work services such as assessments, investigations, diagnosis and treatment planning
  • Provide excellent, evidence based social work interventions to our clients and their families or carers including comprehensive/Psychosocial assessment, advocacy, referrals, counselling and care plans
  • Evaluate the effectiveness of treatment plans and modify as required
  • Participate in care conferences and liaise with other health care professionals as needed
  • Educate the general public and community workers in relation to our services and maintain waitlists
  • Coordination of services

 

About You:

  • Tertiary qualifications in Social Work and the relevant registration
  • Experience in the provision of social work services with adults/elderly favourable
  • Excellent communication skills
  • First Aid & CPR certificate
  • Queensland Provisional or above drivers licence
