An exciting opportunity has risen within our company for a part-time (one day a week) Support Specialist (Social Worker) to join our innovative, multidisciplinary practice. You will be supporting the families of the service to assess their needs and provide support with the following:
- Support to understand children’s, young people and adult’s developmental history and current support needs
- Support to access options for assessment and intervention including Public health and community services and private providers
- Support to understand funding options including Medicare, private health and NDIS
- Support to find relevant allied health professionals, paediatricians and psychiatrists for the child, young person or family
- Support to find support coordinator services or psychosocial recovery coaches
- Support to navigate the process of finding support workers
- Support to discuss complex concerns with school/kinder staff and arrange care team meetings where appropriate
- Support to understand NDIS plans and process
- Liaise and collaborate with PEKE staff to support children, young people and their families
- Build networks in the community to enable referrals as needed
In order to be successful in this position you must be or have the following:
- Qualifications in either social work, social welfare, social science or similar. Or experience within NDIS as a local area coordinator or ECEI coordinator or support coordinator
- Excellent communication, time management and interpersonal skills
- Ability and preparedness to develop the Support Specialist role
- Experience in Allied Health and/or Community Health sector
