Social Worker – Older Persons Community Mental Health SA Health

JOB DESCRIPTION

  • Northern Adelaide Local Health Network, Northern Community Mental Health, Modbury
  • Salary: AHP2 – $85,665 – $99,241 p.a. – Full-Time Temporary – up until 12 September 2023 

The Social Worker (AHP2) will be accountable to the Team Manager of NALHN (Northern Adelaide Local Health Network) Older Persons Mental Health Service (OPMHS), for the provision of social work assessment and interventions for consumers with current positions across both community and inpatient sites. The social worker, in collaboration with the multi-disciplinary team works to optimise health outcomes and to ensure a high standard of consumer-centred care and safe and timely transfer of care to primary health. 

In this role you will apply clinical experience, knowledge and professional competence to plan, implement and evaluate comprehensive and integrated Social Work services to the needs of consumers of OPMHS. You will work under clinical supervision and may provide direction and support to less experienced staff, allied health assistants and students. You will work collaboratively with the multi-disciplinary team and liaise directly with health services and community agencies. You will be accountable to the Principal Social Worker through the Senior Social Worker, for your professional practice and development.

You must hold an appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.

COVID-19 vaccination is an essential requirement under the Emergency Management (COVID-19) (Healthcare Setting Workers Vaccination) Directions 2022 No 7 to work in SA Health/health care setting.
