About the role:
We have an exciting opportunity to work in a dynamic Social Work Department leading our approach to the provision of quality care to clients of the Palliative and Supportive Care Service at SVHS. This position will provide essential bio- psychosocial skills, knowledge and interventions across Palliative and Supportive care to the patients , families carers and staff across the following settings: inpatients, community, outpatients and via the consultation service.
The role will be full time, working with both a dedicated interprofessional and social work team predominantly working Monday to Friday and on call post competency training. The successful Social worker will be addressing life limiting and end of life issues with patients, carers, families as well as staff. The role will encompass advanced communication skills, the provision of education for patients, families and staff, counselling, advocacy, facilitating advance care plans and conversations, assisting with complex psychosocial circumstances and navigating appropriate transfer of care and case management.

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia
2. Bachelor of Social Work from a recognised tertiary provider or equivalent tertiary qualification with eligibility for membership of the Australian Association of Social Workers (AASW) preferably with clinical palliative care experience in a hospital or health related field as a practising Social Worker.
3. Demonstrated knowledge, advanced clinical skills and experience in life limiting illness in inpatient and community based palliative and supportive care, including therapeutic counselling and care of patients, families and carers.
4. Demonstrated high level clinical skills and knowledge in comprehensive psychosocial assessments, crisis intervention, counselling, trauma informed therapy interventions, complex case management, end of life care, advocacy and discharge planning.
5. High level time management, prioritisation, negotiation, interpersonal, written, oral and computer skills.
6. Demonstrated commitment, experience and ability to work as a member of multidisciplinary and social work teams in a specialised environment, in consultation and as an on call social worker.
7. Sound knowledge of legislative requirements and the social work role in regard to domestic violence, elder abuse, child protection, guardianship, NDIS and victims of crime.
8. Ability to undertake and lead clinical planning, research, quality and education projects within social work services as well as the provision of clinical supervision for staff and students.
